Repair/ Return Form
This form should be included with any items shipped back to CTS for repair. You will also need to obtain an RMA# from a customer service representative prior to shipping.
Satellite Service Request Form
Should you require on-site assistance with your products, you will need to request satellite services by filling out this form and faxing it to (970) 667-1032. A Customer Service Representative will call you to confirm the details of your request.
